Why to Use a Form OPM-71?

In the U.S. each person who is legally employed can ask an employer for a temporary leave. For this purpose he/she should prepare certain document that can confirm such absence. People who are employed on federal positions have to submit a form OPM-71, Request for Leave or Approved Absence. How to Complete OPM-71 Form?

To request for temporary leave from a federal position or apply for an approved absence you have to provide the following information in a form template:

  • requester`s full name;
  • 4 last numbers of SSN;
  • name of organization;
  • type of leave/absence;
  • date, time and total hours;
  • purpose of absence;
  • employee signature;
  • current date.

The completed OPM-71 should be further submitted to a supervisor. Later, a signed document will be returned to a person indicating whether a request has been approved or denied. In case of disapproval it is required to specify the reason. Note that any submissions for leave and the decisions are subject to the Privacy Act, but can be submitted to further government review as needed.
